• Fill out visa application form  - original sign 

• two (2) photos Passport size (45mm x 35mm)–photos must be pasted onto application form-original

• Philippine Passport - original(Valid for 6 months)

• Certified true copy of birth certificate issued by the Philippines Statistics Authority (PSA). In case of no record of birth at PSA, birth certificate by city hall and authenticated by PSA as well as certificate of non-record of birth by PSA.  The applicant, who can submit the passport (including old passport) showing the fact that he/she obtained Japanese visa and visited Japan in the past, will be exempted to submit birth certificate. (TAKEN FOR THE LAST 6 MOS.) original

• Marriage contract (PSA)- (TAKEN FOR THE LAST 6 MOS.) - original

• Itinerary for travel – including information as to probable date of entry and departure, name of hotels, addresses, tel. nos. etc.

• Documents to certify applicant’s occupation/profession – e.g. certificate of employment, school certificate, copy of business registration. – original 

• Documents to support financial means for travel expenses – e.g. copy of income tax return (ITR  if employed must be signed by employer and employee / If with Business must be new and stamped by BIR) , and bank certificate (with AVERAGE DAILY BALANCE / if bank certificate don’t have ADB indicated kindly provide additional bank statements at least 3 months) (if dependent, submit supporter, husband or parents documents) - original

• Guarantee letter w/ original sign on middle right side (seal portion)

• Authorization letter

• Original ACR / ICR (Alien)

• Personal appearance for foreign national and applicant w/ Japanese guarantor

• All documents must be in A4 size paper
  
  

Additional Requirements If:

• If Birth Certificate is unreadable or has incomplete information, please provide a birth certificate issued by the Local Civil Registrar.

• If Birth Certificate is “LATE REGISTRATION” Please submit in addition a Baptismal Certificate and School Record (Form 137) From high school.

• Visa interview: Yes/No

• Submission is strictly by appointment, please send a copy of passport

• No appearance and biometric required

• An authorization letter is needed and submit together with the supporting documents

• Estimated waiting time for – approximately 10-15 working days upon submission to the embassy

•  Due to the increasing number of visa applications, it may take several weeks to process an application for a tourist visa. Therefore, we recommend that you apply well ahead of your travel date.
